Asking patients', clinicians and community to get a photo taken with a poster with points on their best advice for type 2 diabetes wellbeing. Will be coordinated into a video for broadcasting on social media, websites.
The Care Partnership - Diabetes (CP-D) program is designed to improve health outcomes for people living with type 2 diabetes (T2D) in Western NSW Local Health District region and Far West Local Health District region. CP-D services support improved patient access to clinical care, professional development for health practitioners, GP practice and ACCHS-led services; and health system improvements.
