Screening for type 2 diabetes Start Date: November 9, 2024End Date: November 17, 2024
Objectives:
Screen 500 individuals for Type 2 diabetes.
Educate participants about diabetes complications and prevention.
Target Population:
Individuals with BMI > 25.
Duration: 9 days.
Location: High-traffic public places (Malls, community centers, parks) to reach a diverse audience.
Screening Process:
Setup:
A small welcoming desk with informational materials.
Ensure privacy for screenings.
Screening:
Use blood glucose meters for quick testing.
Record BMI and blood glucose levels.
Education:
Provide 2-page brochures approved by HCP on diabetes management and prevention.
Offer one-on-one discussions or group sessions on:
Complications of diabetes (CKD, heart disease, nerve damage).
Lifestyle changes (diet, exercise, weight management).
Promotion of the event:
Utilize social media, and community bulletin boards to advertise the event.
Partner with local health organizations for broader outreach.
Follow-Up:
Encourage to those initially screened type 2 diabetes, to follow up with their healthcare providers for additional screening if necessary.
Evaluation
Gather feedback on the educational materials and overall experience.
By executing this plan, we will not only screen for diabetes but also raise awareness about its prevention and management among the community.
